Overview

Provides Evaluation and Determination services in accordance with New Jersey Code and State law (PL 193) and Catapult Learning policy and procedures.  Is the Case Manager and member of the Child Study Team through the process of creating Individual Service Plans (ISPs), and provides Social Assessments as needed.  Assignment will include nonpublic schools in Burlington as well as Florence.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.  Other duties may be assigned.

  1. Attends Assessment Plan meeting for a student if assigned by the Child Study Team Manager.
  2. Completes student assessment, creates report, and submits to Child Study Team Nanager per Catapult Learning procedures.
  3. Maintains caseload in compliance with state timelines and other state and company guidance.
  4. Completes Case Management responsibilities as assigned.
  5. Attends Eligibility Conference for Initial and Re-evaluations if requested.
  6. Completes all required paperwork in accordance with mandated timelines.
  7. Works in compliance with the Catapult Learning Policy and Procedure manual for  Evaluation & Determination (E & D) services in New Jersey.
  8. Remains current with regard to NJ Code and procedures regarding E&D services      provided to nonpublic school students.
  9. Other responsibilities as assigned.

Qualifications

EDUCATION/EXPERIENCE:

Must have Master’s Degree and be certified by the State of New Jersey as a School Social Worker.  Criminal background check as required by the state and the district.  Experience functioning as a member of a Child Study Team in New Jersey is required.

  

OTHER JOB REQUIREMENTS: Ability to work independently and remain focused on task completion.  Excellent ability to interact and communicate with students of all ages, parents, and school personnel.  Strong written communication skills.  Excellent motivation and ability to complete a multi-step process involving many participants.    Excellent organizational skills.  Computer access and literacy to be able to use relevant software.

 

PHYSICAL ENVIRONMENT/WORKING CONDITIONS:

Light lifting, walking, climbing stairs. Most programs are in school settings.
